Terms & Conditions for Tabay & Rosales Dental Appointment System

Please read these Terms & Conditions carefully before using our online appointment booking system and availing of our dental services. By scheduling an appointment, you agree to be bound by these Terms & Conditions.

1. Acceptance of Terms

By accessing or using our dental appointment system, you agree to comply with and be bound by these Terms & Conditions. If you do not agree to these Terms & Conditions, you may not use our services.

2. Appointment Booking and Payment

Appointment Fee: A non-refundable appointment fee of Fifty Philippine Pesos (PHP 50.00) is required to confirm each scheduled dental appointment

Payment Method: The appointment fee must be paid through G-Cash at the time of booking.

Confirmation: Your appointment will only be confirmed upon successful receipt of the PHP 50.00 appointment fee. You will receive a confirmation message/email once your booking is successful.

No-Show/Late Cancellation:The PHP 50.00 appointment fee is non-refundable in cases of no-show or late cancellation (less than 24 hours prior to the scheduled appointment time).

Rescheduling: You may reschedule your appointment twice without incurring an additional PHP 50.00 fee, provided you do so at least [Specify rescheduling notice period, e.g., 24 hours] prior to your original appointment time. Subsequent rescheduling or rescheduling within the notice period may require a new PHP 50.00 appointment fee.

3. AI-Powered Service Suggestion and Data Usage

AI System: Our dental appointment system incorporates an Artificial Intelligence (AI) component designed to analyze your historical dental records and medical information to provide personalized and accurate service suggestions.

Purpose of AI: The AI's purpose is to assist our dental professionals in offering the most relevant and effective treatments and recommendations based on your unique dental history and needs. It is intended to enhance the quality of care you receive.

Data Collection and Use: By using our services, you explicitly consent to the collection, storage, and processing of your dental history records, medical information, and any other relevant data you provide for the purpose of AI analysis and service suggestion. This data will be used solely to improve the accuracy and relevance of the dental services recommended to you.

Confidentiality and Security: We are committed to maintaining the confidentiality and security of your personal and health information. All data processed by the AI system will be handled in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) of the Philippines and our strict internal data privacy policies. We implement appropriate technical and organizational measures to protect your data from unauthorized access, disclosure, alteration, or destruction.

Human Oversight: Please be aware that the AI's suggestions are recommendations only. All final decisions regarding your diagnosis, treatment plan, and services will be made by a qualified dental professional in consultation with you. The AI is a tool to assist our dentists, not to replace their professional judgment.

No Diagnostic Claims by AI: The AI system does not provide medical diagnoses or prescribe treatments. Its function is limited to data analysis and service suggestion based on your provided history.

Data Anonymization/Aggregation: For research, development, and improvement of our AI system, your anonymized and/or aggregated data (where individual identification is not possible) may be used. This will not include any personally identifiable information.

4. Patient Responsibilities

Accuracy of Information: You are responsible for providing accurate, complete, and up-to-date dental and medical history information to us. Inaccurate information may affect the AI's suggestions and the quality of care you receive.

Compliance with Instructions: You agree to follow the instructions and recommendations provided by our dental professionals.

Punctuality: Please arrive on time for your scheduled appointment. Late arrivals may result in a shortened appointment or rescheduling.

5. Clinic's Rights

Right to Refuse Service: We reserve the right to refuse service to anyone for any reason at our discretion, including but not limited to, disruptive behavior, repeated no-shows, or non-compliance with these Terms & Conditions.

Modification of Services: We reserve the right to modify or discontinue any part of our services, including the AI system, at any time without prior notice.

Changes to Terms & Conditions: We may update these Terms & Conditions from time to time. The most current version will always be posted on our website/platform. Your continued use of our services after any changes signifies your acceptance of the updated Terms & Conditions.

6. Limitation of Liability

Tabay & Rosales dental shall not be liable for any direct, indirect, incidental, special, or consequential damages arising out of or in any way connected with the use of our services, including but not limited to, reliance on AI suggestions, scheduling issues, or inability to obtain an appointment.

While we strive for accuracy, we do not warrant that the AI system will be error-free or that all service suggestions will be suitable for every individual.
